Complete the HISPANIC AMERICAN READ-IN CHAIN HOST REPORT CARD found in the white book. Please print this form and make a copy of the report card before you mail it to IRA by October 1. Attach the copy to this WSRA form and mail it to your Zone Coordinator by March 1.
